Carpal Tunnel Surgery – Frequently Asked Questions
Q: What is carpal tunnel syndrome?
A: Typically, carpal tunnel syndrome happens when the median nerve in your wrist becomes compressed. This can cause tingling, numbness, pain, or weakness in your hand and fingers, especially at night or when using your hand.
Q: What are the symptoms?
A: Typical symptoms include:
Tingling or “pins and needles” in the thumb, index, and middle fingers
Numbness, especially at night
Hand weakness or difficulty gripping objects
Pain in the hand or wrist that can spread up the arm
The diagnosis is easy when these typical symptoms are present. However, carpal tunnel is notoriously known to masquerade and mimic other conditions when the symptoms are atypical. For example, It can cause most pain in the top of the arm in the shoulder or elbow and sometimes affect unusual areas of the wrist or hand. This is called “Referred Pain”, making the diagnosis more difficult.
Q: What causes it?
A: Almost 3 in 4 cases of carpal tunnel happen without a reason. Sometimes, it is caused by pregnancy, injury, rheumatoid arthritis, under active thyroid and anything that increases pressure in the wrist. Repetitive hand use can “aggravate” symptoms. There is a predisposition for carpal tunnel syndrome in the Diabetic population.
Q: Do all cases need surgery?
A: No. Many people find relief with wrist splints, activity changes, or steroid injections. Surgery is usually recommended if symptoms are severe, persistent, or interfering with daily life.
Q: How much is the Surgery?
A: It is £900 for surgery. You will need to have an Initial Consultation Appointment with Mr Bhat at a cost of £230 prior to this to confirm the diagnosis, determine if surgery is the right next step and discuss the details of surgery and the recovery.
Q: Why is this cheaper than hospital-based carpal tunnel surgery?
A: When you are seen in a hospital for this procedure there are more running costs involved including the un-necessarily expensive costs of the theatre and all the staff that run it. The Covid-19 pandemic encouraged an innovative new technique called WALLANT to be used to deliver local anaesthetic procedures in an out-patient setting with Mr Bhat and one assistant. Mr Bhat has treated around 300 patients with this new method.
Q: What is WALANT?
A: WALANT is short for "Wide Awake Local Anaesthesia No Tourniquet”. Traditionally, hand surgery is carried out with a tourniquet cuff on the upper arm. This is to reduce the blood flow to the hand and allow the surgeon to see the structures in the surgical wound better. Tourniquet machines are designed to be used only in the main operating theatre for in-patients. However, we can now use Adrenaline with the local anaesthetic injection which is very effective at stopping the bleeding. Therefore, it eliminates the need for a tourniquet and allows you to be treated just as effectively as an out-patient. What is more, you don’t experience the tourniquet discomfort!
Q: What does the surgery involve?
A: Mr Bhat makes a small incision in the palm near the wrist and releases the ligament that is pressing on the median nerve. This relieves the pressure and helps symptoms improve.
Q: How long does the operation take?
A: The procedure usually takes 15–30 minutes.
Q: What type of anaesthetic is used?
A: Carpal tunnel operations are done under local anaesthetic, so your hand and wrist are numbed but you remain awake.
Q: Do I have to stay overnight?
A: No. Carpal tunnel surgery is an out-patient procedure. You will be in the Clinic for less than an hour.
Q: How long is the recovery period?
A: Light activities: usually within a few days
Stitches: Mr Bhat uses dissolvable, invisible stitches
Driving: usually after 1–2 weeks (once you feel safe to control the wheel)
Heavier use or manual work: usually takes 6 weeks
Full strength: often returns over several weeks to a few months
Scar discomfort: the scar continuously remodels over a 12 month period. During this period, you will be aware of the surgery and experience some discomfort.
Q: Will I need physiotherapy?
A: Some people benefit from hand exercises or physiotherapy to restore movement and strength, though many recover well without formal therapy.
Q: Do I have a follow-up appointment and who is it with?
A: At WLC, we offer a routine post-operative 30 minute appointment with a very experienced hand therapist, 7-14 days after your operation. This will cost £100 and is well worth it. The hand therapist specialises in helping prevent important complications such as “pillar pain” and “Complex Regional Pain Syndrome”, helping you make a quicker and uneventful recovery.
Q: What are the risks of surgery?
A: Risks are low, but may include infection, stiffness, prolonged tenderness around the scar (pillar pain), nerve damage, recurrent symptoms, Complex Regional Pain Syndrome (CRPS) or persistent numbness if the nerve was badly compressed before surgery.
Q: How successful is the surgery?
A: The majority of patients notice significant improvement or complete relief from symptoms especially if it is not too advanced. In particular, pain relief is immediate, allowing you to sleep without pain. In advanced cases, some numbness may remain, but surgery generally stops the condition getting worse.
Q: Will both hands need surgery?
A: If you have not received a lasting improvement from splints and steroid injection to both hands, then they will both require surgery. Mr Bhat will decide at your consultation if it’s best to have one hand done at a time or both hands together.
Q: Do I need a referral from my GP?
A: No. You can book directly with our Clinic for an initial assessment and treatment.
